Please complete this form as completely and accurately as possible to allow us to proceed with your pet’s care.

Dear Client,

Your pet is scheduled for an upcoming surgery, and we want to make the day as smooth and stress-free as possible for both you and your pet. To help with preparation, we have provided a few important forms for you to review in advance. These can be found on our website:

  • Pre-Anesthetic Blood Testing Information Form
  • Surgical Information Sheet

Please review these forms carefully. If you have any questions, our team is happy to help—just call us at (408) 215-6344 or email abelvethospital@gmail.com. On the day of surgery, you will also be asked to review and sign an Authorization/Estimate Form, and we will confirm the best phone number to reach you while your pet is in our care.

Before Surgery

  • Please do not give your pet food or treats after 10:00 PM the night before surgery. Water may be left out overnight.
  • If your pet is currently receiving medications, vitamins, or injections, please withhold the morning dose unless you have been given other instructions by the doctor.

Day of Surgery
At check-in, the doctor will answer any remaining questions and collect the signed Authorization Form. If you elected to have the recommended bloodwork performed, this will be completed prior to surgery (if not already done). Should the doctor need to discuss anything further, you will be contacted at the number provided.

After Surgery
Once your pet’s procedure is complete, the doctor will review discharge instructions with you both verbally and in writing. Payment will also be processed at this time. Please don’t hesitate to ask for clarification on any instructions, either during your visit or afterward—we want you to feel fully supported in your pet’s recovery.
